Care Coordinator Job Description: 

Summary of primary position functions:

The Care Coordinator will provide families enrolled in the PNCC/CCC coordination programs with case management, life skills development, infant/parent relationship support and family support services to enhance the physical, social, emotional, and intellectual development of participating families.


Duties and responsibilities

Provide information and support about prenatal, postnatal, and well-baby care
Provide links to appropriate community resources
Provide support services and referral to health and human services for pregnant women and their families at risk for poor pregnancy outcomes due to physical, social, psychological, financial, or environmental factors.

Minimum Qualifications

Associate Degree in Human Services, Health Care, or Social Services. Preferred 2 years of training and experience in: infant/toddler development or early childhood education.

Knowledge of local community resources and the ability to refer and connect families with appropriate agencies and services

Experience in home visiting and case management

Has a strong passion for helping others and improving the community.

Superior written and verbal communication skills, ability to establish rapport.

Have the ability to work a flexible schedule (some evening work may be required)

Excellent time management skills.

Good documentation skills.

Bilingual (English & Spanish)/Multilingual a PLUS.
